The Office of Human Resources coordinates compliance with the Americans with Disabilities Act Amendments Act (ADAA) and supports employees and applicants in accessing reasonable accommodations. Determining accommodations is a collaborative process involving HR, the employee, their supervisor, and other relevant staff.
Who Is Protected
Under the ADAA, a person with a disability:
- Has a physical or mental impairment that substantially limits one or more major life activities,
- Has a history of such an impairment, or
- Is regarded as having such an impairment.
A qualified individual with a disability is someone who meets the skill, experience, education, and other job-related requirements of the position and can perform its essential functions, with or without reasonable accommodation.
Requesting an Accommodation
Employees seeking workplace accommodations are encouraged to contact HR directly. You may discuss your needs with HR without involving your supervisor initially; supervisors will be included later in the process.
Steps to Request an Accommodation:
- Submit an Online Request – Complete the Employee Accommodation Request Form. Provide details about your role, your medical condition, your contact information, your supervisor’s contact information, and your accommodation needs.
- Documentation – Your licensed healthcare provider must complete the Employee Accommodation Medical Certification Form, documenting your limitations and expected duration. You may request a copy of the form by contacting HR.
- Interactive Dialogue – HR will work with you to explore accommodations that enable you to perform your job effectively while maintaining essential job functions. Alternative solutions may be suggested if your initial request is not feasible.
- Finalize the Request – Approved accommodations will be documented in a written letter.
Confidentiality
All medical and accommodation information is confidential, maintained separately from personnel files, and shared only on a need-to-know basis.
