What Employees, Managers, and Supervisors Should Know about Workers’ Compensation Reporting. We encourage you to review the following helpful documents included in your packet:

If You Do Not Wish to File A Workers’ Compensation Claim:

Once you have completed all of the Workers’ Compensation Claim documents. If you decide not to pursue a Workers’ Compensation claim, please respond to the original email with the following statement:

  • “I, <Your Full Name>, the injured employee, choose not to file a Workers’ Compensation Claim for the injury reported at this time. I understand that this decision will halt all further claim processing by the ACC Risk Management Department. If I decide I want to file a claim in the future, I must complete all necessary requirements listed above to file a Workers’ Compensation Claim.” Please attach an image of your ACC Badge/ID in your reply to serve as your signature.
