I. Immediate Response Guidelines
Evaluate the situation! Quickly determine if you are in immediate danger, and note the level of injuries. Ensure your own safety first! Once you are safe, please respond accordingly for life threatening danger/injuries or non-life-threatening incidents/injuries.
- Life-Threatening Danger and Injuries: Call 911 or 3-1231 immediately from any ACC desk phone to contact the ACC Police Department as first responders. If using a cellphone, call (512) 223-1231. Clearly state your location, nature of emergency, and any injuries.
- Non-Life-Threatening Incidents: Notify ACC Police at (512) 223-1231. Provide clear details about the situation and your location
II. Reporting Procedures
This section provides a step-by-step walkthrough of the ACC Risk Management reporting process. By following these steps, we can ensure a safe and secure environment for everyone at ACC. Stay safe and report all incidents promptly!
- Step 1: Depending on the type of accident, complete and submit the appropriate Risk Management accident form immediately after an accident:
- Employee Accident Report – For ACC employees with a work-related injury.
- Student Accident Report – For accidents involving students actively participating in an ACC affiliated course/activity.
- Auto Accident Report – For accidents involving ACC-owned or operated vehicles.

- Step 2: As soon as you receive the confirmation email after submitting the accident form, promptly forward a copy to your immediate supervisor, department head, and any other relevant stakeholders. Include a brief summary of the accident details to ensure they are informed and aware of the situation.
- Step 3: Within 24 hours submit the SEM Injury/Incident Reporting Form for all ACC related incidents.
III. Roles & Responsibilities
During or after an emergency, please be aware of what is expected of you depending on your role.
- Employees: Any employee other than the injured individual is responsible for immediate reporting using the relevant forms. This ensures the injured party has all the documents they need to seek care through ACC’s insurance and start an insurance claim.
- Supervisors: Ensure your employees understand when and how to report accidents accurately and promptly.
- ACC Police: Immediate emergency response, securing the scene, initial investigation, and First Responder reporting.
- Risk Management: Review and manage accident claims and implement mitigation processes.
FAQ
What if I'm unsure whether an accident is reportable?
Every accident should be reported. What seems like a minor injury could become more serious without warning after the initial shock or adrenaline has passed. Follow the steps above to report the accident.
How soon should I report an accident or injury?
Immediately after it is determined that the employee or student is stable.
Who should fill out the accident report?
An ACC Employee other than the injured employee. Instructor, Supervisor, Campus Police, Campus Management, any other ACC Employees available to assist the injured person.
What if an accident occurs after normal business hours?
Follow the steps above to the best of your ability. Contact your Supervisor for immediate assistance, or the next business day contact Risk Management for assistance.
What if the injured party is both a Student and an ACC Employee?
The type of claim will depend on what role the student is operating in at the time of the injury.
- If the injured party is injured during their role as an ACC employee (during work hours, attending an employee event), fill out an Employee Accident form.
- If the injured party is injured during their roll as a student (in class, attending an event as a student, etc.), fill out the Student Accident form.
